Short term use of medication

The school will not give your child medicine unless you complete the form below. Please note that by completing the form you are agreeing to the following:

"I agree to members of staff administering medicines, first aid and emergency treatment to my child as directed below or in the case of an emergency, as staff may consider necessary. I understand that it is my own responsibility to ensure the medicine is taken to the school admin office and handed to a member of staff on arrival at school and that the medication is collected at the end of the school day if it is required at home.

I accept that this is a service which the school is not obliged to undertake. I recognise that school staff are not medically trained.

The above information is, to the best of my knowledge, accurate at the time of writing and I give consent to school/ staff administering medicine in accordance with the school/setting policy. I will inform the school/setting immediately, in writing, if there is any change in dosage or frequency of the medication."

NB: Medicines must be in the original container as dispensed by the pharmacy (this is not the responsibility of CHS staff).
